3 Easy Steps - Get Application Status Report
Here are 3 easy steps for requesting 'Detailed Application Status Report':

Complete IMM 5475 - "Individual's Consent to Disclosure Form"
Please note that each of this form is required for any adult individuals (over 18 years of age). If you or your family is sponsored, then the sponsor also needs to complete this form.

- After "I, the undersigned,", put your full name
- After "born", put your date of birth (preferably in format like "1-Jan-1964") to avoid ambiguity
- After "located in my file #", put your immigration file number (normally "B" followed by 9 digits), for instance "B 123 456 789"
- after "in", put the visa office. Just the town will do, for instance "Buffalo", "London", "Moscow", or "Accra"
- after "Name (printed)", at the bottom, put your full name in capitals
- after "Date", put the current date in the boxes provided
- after "SIGNATURE", sign in the box provided

Complete our Online Application Form.

Mail your 'Consent to Disclosure' (Point 1) along with a bank draft with the appropriate fee depending on the type of service you choose. Please make your bank draft payable to "United Services Canada".
You can fax your form to 1-905-286-9114.
If you wish to pay by wire transfer, our account details will be provided.
